Shocking waiting lists across the UK for ADHD diagnosis

NICE Guidelines state that no one should wait longer that three months between being referred for an ADHD Assessment and being seen, but from the abundance of media coverage and statistics out there, it is clear this is sadly not the case across the United Kingdom. We looked at some of the wait times and the results are shocking. Here are just a few examples:

Leeds and York NHS Trust

The Leeds and York NHS website advised last month that they typically receive 195+ referrals per month which exceeds their capacity by far. As of January 2024, they noted approximately 4,000 people on the waiting list. Those being assessed currently joined the waiting list in January 2021 – a 3 year wait.

They further note that “This waiting period is accurate for those at the front of the waiting list but will be several years longer for those who have been referred more recently. Due to the gap between demand and capacity, this waiting time is expected to increase.” Diagnosis is not the only worry; Leeds and York NHS Trust further report that there were, in January 2024, approximately 290 people on their ADHD medication waiting list; those being booked currently having waited since December 2021, a staggering 2 years and 4 months wait. This is an excruciatingly long wait between receiving diagnosis and being started on medication.

Devon NHS Trust

Advice on the Devon NHS website, updated in February 2024, states they are seeing people who were referred in July 2021 – a wait time of approximately 2 years and 7 months. A slight reduction in comparison to Leeds and York NHS, but only marginally. On the plus side, the wait for medication is slightly lower promising 18 months with those being referred in October 2022 currently being seen.

Central and Northwest London NHS Trust

The Central and Northwest London NHS website advises that since November 2023 they have had to close their waiting list.  Due to receiving extremely high numbers of referrals they are at capacity. Astonishingly, their current wait time is over 36 months, for those that managed to get on the waitlist prior to closing.

It was reported by the BBC in March last year that “Assessments for attention deficit hyperactivity disorder (ADHD) can take longer than five years in Sussex and up to four years in Surrey, data from health bodies show.”

In the last couple of weeks the BBC have reported “NHS cannot meet autism or ADHD demand, report says”. Quoting an average of one to two year wait times for ADHD assessments for 24% of patients and a 51% increase in demand for prescriptions for ADHD medications since 2019.

Autism Spectrum Disorder Assessments

As well as looking at the significant waiting times for ADHD assessments, it is important to highlight the significant waiting times for Autism Spectrum Disorder assessments. According to the statistics, 50-70% of individuals that are diagnosed with ASD also have ADHD present. This translates into approximately 25% of those diagnosed with ADHD also having ASD; although it is thought between clinicians that this is much higher, yet undocumented.

The latest NHS figures report, the highest ever recorded figure, 172,000 adults and children that are on waiting lists for an ASD assessment and it seems reasonable to assume that this is only set to rise based on the growth in figures over the last few years. With the report “A country that works for all children and young people: An evidence-based plan for addressing the autism assessment and support crisis” reporting that since Covid-19 there has been a 306% increase in the number of children waiting for an autism assessment.

The Devon NHS website advises that, as of February 2024, the current wait time was approximately 5 years and 6 months for an ASD Assessment, with patients being initially referred in August 2018 currently being assessed. With NHS ‘experimental’ data showing that, as at June 2023, 143,119 people were waiting for an autism assessment. With this number rising to 157,809 on the waiting list by September 2023. The September report also depicts a worrying picture, noting that the number of diagnoses were actually down by a third against the previous year. With only 754 patients being diagnosed in the month of September 2023 in comparison to 1,058 in September 2022. With a 27% rise in new referrals being reported over the same period it is difficult to see these numbers reducing in the near future.

The Nice guidelines timeframe of being seen within 3 months of referral is completely lost here, with only 6.4% of patients getting an appointment for assessment within 13 weeks of referral being reported in the September 2023 statistics.

The request for evaluations for autism and ADHD has surged so rapidly that existing services are overwhelmed. Heightened recognition of neurodiversity, combined with staffing challenges, the effects of the pandemic, and persistent financial shortfalls, have resulted in escalating delays and extended wait periods that detrimentally impact individuals’ daily experiences.

The consequences for children are profound. Lacking an official diagnosis, children and adolescents who may have autism or ADHD often do not receive necessary assistance in educational settings, potentially altering their future path significantly.

This eye watering situation is part of the reason why we opened The ADHD Clinic. For adults, we offer two tiers of assessment.   Tier 1 is a multi-disciplinary assessment. The client has an appointment with a psychologist, and then an appointment with a psychiatrist, who will complete a joint report giving a comprehensive and nuanced perspective.

Neurodevelopmental difficulties are largely cognitive constructs diagnosed based on (mainly) observable behavioural symptoms. In simple terms – neurodevelopmental difficulties are problems with how our brain develops and we figure this out mainly by looking at how people behave in certain situations.  A Psychologist can carry out various psychometrics and can also complete cognitive assessments to look at impairments associated with ADHD. Their extensive training in behavioural and cognitive theory allows for a more in-depth evaluation, ensuring a holistic understanding of an individual’s challenges.

A psychiatrist, with their medical training, brings a unique viewpoint to the assessment, examining aspects from a clinical and physiological standpoint.  A psychiatrist is medically trained and will undertake a structured medical assessment of your mental health and psychiatric history. This means that the doctor will look at other health conditions, especially mental health conditions, which may either exist alongside possible ADHD or exist instead of ADHD.  For example, if people have depression and/or anxiety that is not responding to standard treatments, sometimes this is because ADHD is present and needs to be treated first to enable other conditions to be treated subsequently.

The collaboration between a psychologist and a psychiatrist, (supplemented by input from a trusted, familiar person, and the completion of targeted questionnaires) is exceptionally thorough.  It offers patients a more detailed understanding of their presentation, diagnosis, behaviours, and cognitions, enhancing the overall quality of care and paving the way for a tailored intervention plan.

This thorough approach facilitates personalised support for your journey; a psychologist making in depth and specific recommendations for work and/or education focusing on the social emotional and practical difficulties associated with ADHD; a psychiatrist recommending and prescribing medication based on an assessment of its potential benefits, diligently monitoring its effectiveness, and observing progress and improvements to ensure optimal outcomes.

For those seeking a more expedited process and a concise report, we proudly introduce our tier 2 assessments. In this streamlined approach, assessments are conducted solely by a psychiatrist. While the medical expertise ensures a high standard, it may not delve as deeply as our tier 1 assessments, where both a psychiatrist and psychologist collaborate to provide a more comprehensive perspective. Opting for tier 2 assessments offers a quicker route without compromising quality, catering to individuals who prioritise efficiency in their evaluation process.

In catering to children, our team boasts highly experienced child and adolescent psychiatrists committed to delivering meticulous and comprehensive assessments. Beyond a structured interview, our approach involves a thorough examination of school records, medical histories, and in-depth discussions with parents. The Great British Post Office Scandal has been one of the most shocking stories to emerge in recent years. More than 900 sub-postmasters and postmistresses were wrongly accused of theft, fraud, and false accounting. Many of which have been convicted and jailed. The scandal was caused by faulty software that incorrectly made it look like money was missing from post office branches across the UK.

The webinar featured Nick Wallis, a famous investigative journalist who first broke the story. Wallis discussed the devastating impact the scandal had on the lives of innocent people who had been wrongly accused and convicted.

The Post Office had the power to prosecute, leading to 700 sub-postmasters and sub-postmistresses being prosecuted between 1999 and 2015 based on the Horizon computer accounting system. Additionally, other organizations, such as the Crown Prosecution Service, have brought forward 283 cases. Despite repeated attempts to flag issues with the software. Many were convicted and imprisoned for false accounting and theft, while others were left ruined financially.

After two decades of campaigning, a legal victory was finally achieved, and 93 convictions have been overturned to date. Under government plans, victims can fill out a form stating their innocence to have their convictions overturned and claim compensation. Downing Street has also pledged to collaborate with Scotland and Northern Ireland to ensure that those wrongly accused in those nations can have their names cleared.

Moreover, the Metropolitan Police is investigating the Post Office for potential fraud offenses stemming from the prosecutions. It is worth noting that a public inquiry, which began in February 2021, is ongoing. Victims are still advocating for appropriate accountability and redress.

The scandal has also been brought back into the public eye by the new ITV drama, “Mr Bates vs. the Post Office”. Which has been hailed as the network’s biggest new drama in over a decade. The drama has brought the story of the wrongly convicted sub-postmasters back to the forefront of the nation’s mind.

Despite the pressure from national press, fewer than 100 wrongfully convicted individuals have had their innocence reinstated. However, there is hope that justice will eventually be served. The Met Police has confirmed that it is currently examining a possible conspiracy to pervert justice at the Post Office.

One of the most high-profile figures involved in the scandal has been ex-Post Office boss Paula Vennells. She recently bowed to public pressure and handed back her CBE, saying that she was: “truly sorry for the devastation caused to the sub-postmasters and their families” and that she is continuing to support and cooperate with the inquiry into the scandal.

The effects of the Post Office scandal have been devastating. Particularly on the mental health of those wrongly accused and convicted. More than 700 innocent postal workers were accused of crimes they never committed. Including village post office shop owner Alan Bates, who lost £65,000. Faulty technology caused sub-postmasters across the nation to be sacked, thrown in prison, and hit with immense mental distress. Tragically, four people died by suicide and others spiralled into a deep depression.

Mental health at Xmas: Loneliness

As the festive season approaches, many of us look forward to spending time with loved ones and enjoying the festivities. However, for some, this time of year can be difficult and lonely. With the ongoing pandemic, this year may be even tougher for those who are struggling with their mental health. In this blog, we will firstly discuss the issue of loneliness during the Christmas period. Following by how we can be more aware of other people’s mental health.

According to the National Centre for Mental Health, loneliness can have a significant impact on mental health. During the festive season, when many people are surrounded by family and friends, those who are struggling with loneliness can feel even more isolated. It is important to remember that loneliness can affect anyone, regardless of their age or circumstances.

We believe that awareness is key. It is important to be aware of other people’s mental health – reach out to those who may be struggling. This can be as simple as checking in on a friend or family member or inviting them to join in with festive activities.

It’s important to remember that loneliness is not the only mental health issue that can arise during the Christmas period. Breaking Point – why do mothers hurt their babies?

Postpartum psychosis is a severe mental health condition that can arise suddenly in new mothers. A temporary lapse in sanity that can cause women to lose touch with reality, sometimes with catastrophic consequences.

Research suggests about 20% to 50% of parents will have thoughts of harming their baby at some stage, especially in the first few weeks of life.

For the vast majority, this will remain just a thought – most likely an intrusive, unwanted one at that.  However, for some, they will find themselves hurting their child deliberately, perhaps causing serious harm and even death. The question is why?

The team take on The Dorney Lake Season Finale Triathlon

Fitness and wellbeing are high up on the priority list and having had such a great time participating in The Blenheim Palace relay triathlon in June, several of the team decided it was time to each take on a full triathlon.

Preparing for The Dorney Lake Season Finale TriathlonSo, we devised a (pretty brutal) training schedule, and the office was filled with bikes and running shoes ready to spend lunch hours and evenings practising, and in some cases learning, cycling, running and swimming.  Leigh, Tallula, Nattasha, Shyann and Zofia spent Tuesday evenings driving up to the lake in Lydd to practice open water swimming.  As you know, we haven’t had the best summer so it was often pretty cold in the water, but we always felt great afterwards.  Cold water swimming is definitely as good as ‘they’ say.

The three months we had to train went very quickly and before we knew it, it was September, and time for The Dorney Lake Season Finale Triathlon.

 

Tragically, on the day, Leigh and Tallula ended up in a crash on the M25.  The car was written off but thankfully no one was hurt.  It did however, mean that neither could make it to Eton Dorney to take part in the triathlon and spent the day waiting for recovery in the hard shoulder.  It also means that they can keep training for another triathlon later down the line. Don’t worry, we won’t let you forget girls!

So, in the pouring rain, Zofia, Nattasha and Shyann took on the challenge and all absolutely smashed it! All three completed the lake swim, the cycle, and the run, and in great times. A huge achievement and another medal to add to the collection. Well done ladies!

Why did you decide to take on a triathlon?

For the challenge. I have always thought it was something I would never be able to achieve but you can conquer anything you put your mind to.

What were the main challenges during training? 

Swimming. As someone who never had swimming lessons as a child, this really was the biggest challenge. Learning how to swim correctly in 3 months, and in open water, was the hardest challenge of them all. There were many emotions during the process but seeing improvement each and every time I went swimming really kept me going and I am so thankful I had those who took part to help me learn and push me to keep going.

What did you enjoy most about the experience? If you did enjoy it!

Getting out of the lake as my biggest fear was failing at the first hurdle and needing to be rescued. Once I got that part over with, the immense sense of achievement pushed me through to the end.  Seeing my swim time at the end and doing 1000 times better than I ever thought I would do made me want to do it all over again. I will be back next year to take on the challenge again!

Why did you decide to take on a triathlon?

I always thought of it as an impossible challenge. I wanted to prove to myself that I am strong and I can do anything I put my mind to.

What were the main challenges during training?

The discipline to get to the lake on rainy days, the early starts and the doubts of, can I really do this..?!

What did you enjoy most about the experience?

I loved every single second. It went by so fast – I wanted to do it all over again as soon as it was over…am I crazy?

Why did you decide to take on a triathlon?

I am someone who has never really enjoyed exercise or sport but do love a goal and a challenge.  Not in my wildest dreams did I think I would ever complete a triathlon so this had to go on the bucket list.

 What were the main challenges during training?

Learning to ride a bike properly for a start! The office car park was a good starting point.  Secondly time – fitting in training for three different disciplines while working full time and walking dogs three times a day was a challenge in itself.

 What did you enjoy most about the experience?

To be honest I loved everything about it.  The sense of achievement with each and every stroke, pedal or step, the absolute joy when we completed it, the atmosphere and the shared experience.

So, as we always say, set yourself a goal and/or a challenge.  It really is possible to achieve things you never thought you could.  It just takes commitment, hard work, and encouragement from those around you. And the elation you get from achieving it makes it even more worth it.

The Great Post Office Scandal is the story of how these innocent people had their lives ruined by a once-loved national institution and how, against overwhelming odds, they fought back to clear their names.

Nick Wallis, best known for his work on the Post Office Horizon IT scandal, joins us and we are honoured to announce that he will host this informative and frankly mind blowing webinar on the Post Office Scandal.

On 23rd April 2021, the Court of Appeal quashed the convictions of 39 former Subpostmasters and ruled their prosecutions were an affront to the public conscience.

They had been prosecuted by the Post Office using IT evidence from an unreliable computer system called Horizon. However, when the Post Office became aware that Horizon didn’t work properly, it covered it up.

Nick describes how a group of Subpostmasters worked out what was going on. Formed a campaign group and fought the government-owned Post Office through the courts to eventual victory.

The Great Post Office Scandal has been described as “an extraordinary journalistic exposé of a huge miscarriage of justice” by Ian Hislop, Editor of Private Eye Magazine. Dame Joan Bakewell says “Nick’s narrative has the power of a great thriller as he lays bare the lies and deceit that has ruined so many lives.”

The Importance of Pride Month for Mental Health

Pride celebrations, festivals and marches dominate the calendar in the summer months and especially in July. If you are a member of the LGBITQ+ community, this can stir up all sorts of emotions. Although there is much to be proud of and celebrate, it’s important to remember that there is still progress to be made towards achieving true equality. By pondering what Pride means to us and to our community, we can initiate new dialogue, gain fresh perspectives, and explore new opportunities for growth and learning.

Why we celebrate Pride

Historically, LGBITQ+ identities were considered by psychology professionals to be mental illnesses. It wasn’t until 1990, when the World Health Organization (WHO) removed homosexuality from its list of mental disorders. Unfortunately, to this day, 67 jurisdictions still criminalize same-sex sexual activity, with 42 of those countries prohibiting consensual sexual activity between women. Shockingly, 11 of those countries (https://www.thepinknews.com/2023/05/17/lgbtq-death-penalty-idahobit/) even impose the death penalty for queer individuals. As a result, the LGBITQ+ community has faced threats, abuse, mistreatment, detention, punishment, social exclusion, and even death.

As such, celebrating Pride is an opportunity to highlight the diverse lives of LGBITQ+ individuals and acknowledge the beauty and joy of the community. It’s a way to show the importance of acceptance and inclusion, which is especially crucial in the face of negative and prejudiced messages about LGBITQ+ people.

Pride History

Pride has its own significant history that is important to remember. After all, for many years Pride was effectively also a protest. The first Pride happened in 1970, to mark a year since the Stonewall Uprising, where LGBITQ+ people protested the discrimination and violence they were experiencing from the police.

In 1972, the UK’s first Pride march took place in London. As well as a celebration, it was a protest against the unfair treatment of LGBITQ+ people. Male homosexuality stopped being illegal in England and Wales in 1967, but in 1972 it was still illegal in Scotland and Northern Ireland. Being gay was still classed as a ‘mental illness’ by the World Health Organization (WHO) until 1992 – and they didn’t stop classifying being transgender as a mental illness until 2019.

Society made lots of people feel ashamed of who they were because being gay, queer, or trans was seen as being wrong, which in turn had an adverse effect on their mental health.

For this and many other reasons, Pride protests have been important in securing the rights of LGBITQ+ people across the world. People continue to protest through Pride marches because LGBTIQ+ people still face discrimination and inequality in the UK and around the world.

Issues LGBTIQ+ people face

Being LGBTIQ+ does not automatically mean someone will have mental health issues, but does nonetheless mean that they are at higher risk of experiencing poor mental health, than other societal groups.

As a clear indication of this, a study conducted by Stonewall (Take Pride report 2022) found that over the previous year:

  • half of LGBTIQ+ people had experienced depression, and three in five had experienced anxiety
  • one in eight LGBTIQ+ people aged 18 to 24 had attempted to end their life
  • almost half of trans people had thought about taking their life
  • Hate crime
  • Accessing healthcare

Pride month mental health benefits

There are a plethora of mental health benefits that can be derived from pride month, some of which have also been scientifically documented. So let’s have a look at a list our team have created for you below:

Connection: Several studies demonstrate the link between loneliness, poor mental health and early mortality. As it is often the case. LGBITQ+ people can feel disconnected from friends and family, and find social acceptance difficult to come. Causes such as pride month, enable them to feel a sense of inclusion and community through events, marches, articles and media focus. Even though taking part can initially be intimidating, the relatability, common interests, affirmation and human connection can be a tremendous confidence boost.

Identity: Lack of affirmation and constant abuse and questioning of one’s identity can often lead to a suffering mental health. As such, being alongside others on the same or similar path, can help LGBTQ+ to move between stages and reach a point where they are able to be proud and non-fearful of their identity.

Representation: Living in a society that adheres to hetero-normative and cis-normative standards makes it challenging to find individuals who share the same experiences. When we see others like ourselves represented, it gives us a sense of belonging and the confidence to pursue our goals fully. This can be the inspiration we need to take significant steps in our personal and professional lives, whether it be at school, work, or home.

Role models: Pride is a time of celebration, where we share our stories and listen to those who have overcome hardships to lead authentic lives. These stories can motivate us to take an important step in our journey towards personal pride.

The LGBTIQ+ community faces a higher risk of mental health problems. For example, depression, self-harm, alcohol, and drug abuse, and suicidal thoughts. Pride has been shown to have numerous mental health benefits for LGBTIQ+ individuals and the community as a whole. Not only does Pride help secure equal rights, which is crucial for their well-being. It also provides a platform for people to celebrate the best parts of being LGBITQ+. This includes acceptance, belonging, community, and joy!
